The 5 Stages of ADPIE

  • Assessment: Synthesizing subjective patient reports with objective clinical observations (vitals, lab results, Glasgow Coma Scale).
  • Diagnosis: Formulating precise NANDA-I nursing diagnoses with etiology and defining characteristics.
  • Planning: Establishing SMART patient outcomes (Specific, Measurable, Achievable, Realistic, Timely).
  • Implementation: Executing evidence-based clinical interventions cited with recent peer-reviewed healthcare literature.
  • Evaluation: Reassessing patient response against expected outcomes and adjusting interventions accordingly.
Academic Rigor: For reflective nursing essays, pair the ADPIE workflow with Gibbs Reflective Cycle (1988) or Johns Model of Structured Reflection to demonstrate critical self-awareness in clinical practice.
